Palo Alto Women’s Center is contracted with most insurance plans. If you do not see your insurance plan listed, contact the office to inquire if we participate in your insurance or there is a contract pending.

Please print and fax the authorization of release of medical records in the forms section your previous doctor.
You may also contact the office for additional information on what to do

Please call your Pharmacy who will then send a refill request to our office.
If you need to call the office, please call during the telephone hours so we can check your records to make sure a refill is the best course of treatment. Please allow up to 2 business days for medication refill.
We do not give new prescription is you have not been seen for a condition

You can access the results of lab work through our patient portal
Our policy is to inform patients of abnormal results in a timely fashion. Please inform the office of any changes in your contact information.
Blood test, PAP result and biopsy results are general available in 3-7 days. Please sign up on our patient portal for you lab results. Other Lab results are usually available 2 weeks after the test was performed. Please inquire accordingly.

If you are unable to keep an appointment, please call our office as soon as possible, preferably 24 hours or more before the appointment, so that your appointment can be rescheduled properly

MEDICAL RECORD COPIES:
$20.00 first 25 pages
$0.15 per additional page.
Please allow up 14 business days. Payment must be made before the copies are printed.
FMLA/ADMINISTRATIVE PAPPERWORK:
We charge $25.00 due at pick-up or before forms are faxed. Please allow up to 7 business days for completion.

Definitions related to Insurance

Co-insurance is based on the pre-determined level of coverage outlined in your insurance policy – many commercial insurance companies pay on claims at a level of 80/20, meaning the insurance company will consider and pay 80% of the allowed charges and the remaining 20% is considered the patient’s co-insurance.

A co-payment (or copay) is a fixed, out-of-pocket fee you pay for a covered medical service or prescription drug.
Palo Alto Women’s Health Center is a preferred provider with many third party payers. Many of these insurers require a minimal co-payment for office visits.
If your insurance coverage requires a co-payment for office visits, you will be required to pay this at the time you check-in for your appointment
